I was just noticing today, speaking to an acquaintance I had not seen in a while, how this perceptual distortion can persist in a person even years into middle age. The person is in her middle 50s, married with children, attractive, slender, something of an exercise anorexic now, but normal tho low weight. She talks about herself as a "fat pig" and speaks with disgust about how her size 4s are too tight on her. She is sufficiently distressed as she does so that it is clear she is not just fishing for compliments or reassurance that no, she is not fat. She clearly is sure that she is overweight. My guess is that she was anorexic in youth. However, she does not believe in therapy or meds, and I think she channelled her obsessive energies into other things and substitute addictions (shopping, for example)
