cold is more a trigger for ME than for M…but I’m SUPER vigilant about putting the scarf around her nose and mouth….and I don’t let her out in the freezing cold. She also has a note that states she doesn’t go outside at school if the temperature is below 35 degrees (32 is the district minimum…and most of the teachers won’t go out anyway…so it’s usually a moot point)
She will normally cough a bit, but will usually settle down with a shot of Xop and warming up. I typically end up in a full-fledged flare requiring antibiotics and prednisone
Jumping in late here (computer crashed, so I’m behind on my blogs!), but I was wondering, for those of you who do react to the cold, does just going in to warmer air stop the flare? As well as ventolin/albuterol/xopenex?
My asthma boy sometimes flares with the cold, but he really doesn’t like the way the albuterol makes him feel (and I don’t like how crazy it makes him-but I’d rather have him breathing and crazy than not). I imagine that the warm air would work, but would take longer, which seems to this non-asthmatic like torture. Is that right?
I can’t tell you how much I value the experience of moms of older asthmatic kids and that of asthmatics old enough to pass on their wisdom so that I can better take care of my kids (and any patients in the hospital I may care for). Thanks guys!!
ScienceBlogs is pretty cool. I only discovered it about three or four months ago, but it’s already one of my ‘timesink’ sites – where I go when I’m bored and have time to kill (like when I’m drying a reactant – a boring and time-consuming process that’s mostly ‘hands off’ but I can’t do anything else until it’s done because I’m removing water and the only other reaction I want to do is extremely water-sensitive so water is a no-go on my Schlenk line while I do it – and waiting for a TA meeting to start).
Another timesink is Youtube, specifically the science vloggers there, like AronRa and c0nc0rdance (both of whom have some excellent material refuting intelligent design and c0nc0rdance also dabbles in debunking alt-med and GMO hysteria). I’d definitely recommend them, with the caveat that Youtube vloggers can make Orac look like the epitome of mild manner, gentle disposition and extreme consideration by comparison at times. But, to me, that’s part of the fun. But then, I enjoy that sort of humor.
Amy – that definitely plays a part, too, and I think it’s part of why those who are superstitious hold onto their superstitions so strongly. It gives them something to blame if something goes wrong.
When I was kid and something that I thought was outrageously unfair (like a childhood friend being hospitalized for a severe kidney infection when I was about six), I’d ask my Dad, “Why?”
His response wasn’t comforting, but I think it was right: “Sometimes, honey, bad stuff just happens. It sucks, and it’s no fun. But you can tear yourself up looking for ‘why?’ Instead we can concentrate on turning the bad into something good, or at least something better. That way we’re doing something about it. Why don’t we go visit [friend's name] at the hospital, and you can give him some better food than the yucky hospital stuff, and maybe brighten his day a bit? How does that sound?”
But that said, having an answer to ‘why?’ even if the answer is wrong, can be very comforting.

When AG went through her bronchitis, moving her to warmer air stopped the flare BUT she was already using her inhaler round-the-clock, anyway.
RNMom, I imagine that would be the way for some people. Certainly, if I’m having a very mild cold-induced attack, it works that way for me, but if it’s got going into a full-blown attack and I’m in trouble, then I seriously need my puffer.
It’s the same for any trigger with me. If I’m around a smoker, moving away from the smoke can stop my coughing if i’m just getting started, but if I’ve already gotten going, I need to give in and take the puffer or at the very least I’ll be miserable for several hours until my lungs want to behave. But I’d guess it would depend on the person.

You can do this. It feels so overwhelming when you first get the diagnosis, start doing all the meds, and start researching the heck out of asthma. It will get better. You will become an expert on your kid’s asthma, her triggers, when she needs meds, when she’s doing better. It will seem like it’s taking forever while it’s happening, but it won’t really.
There’s no telling HOW soon it will happen, but it will happen soon. You’ll get used to the new normal. Good luck, and don’t forget to take care of yourself too-when my son was first diagnosed, I spent a lot of late nites up researching and I know I wasn’t at my best the next day. Too many of those and I just ran myself into the ground.

The BAX 3000 laser treatment is working for me. I had been on meds for 33 years, and twice hospitalized. I was skeptical, not really hopeful, but I had tried everything else, and decided to gamble. I signed up for the treatment in late 2009, when it was advertised as available in my area. At first I didn’t see any change, then cut back on the meds, and now I am off the meds. It was not covered by my insurance, but for me, it has been totally worth it. If it is available in your area, you might want to look into it.
She will need to sit still for the few minutes that the treatments take, but it is painless, and she is old enough to follow the simple directions to sit quietly for a few minutes.
Ok…My AG developed hers @ 5 & was sick ALL of the time. For us, it took about a year and a half, but that was due to her triggers. The best thing to do to get control is to find the triggers and minimize/eliminate them if possible, and just figuring out what’s causing the constant flaring can be hard. My AG’s were chronic sinus infections, GERD and allergies. Once we were able to handle these and she was on the maintenance inhaler for many months, her asthma improved. If your AG seems snotty/mucous sick all of the time like mine used to be, I would highly recommend having docs help identify whether it’s allergy, sinus infections and/or a poor immune system so you can start tackling the trigger. Additionally, if she is snotty a lot w/post nasal drip, I would recommend saline nasal rinses – no added meds & they really clear things up, though they’re not fun for kids. I do them with my AG & my own sinus problems have cleared up, too. You might want to run this by her doc first, but they really do help with no side effects.
Why all of the sudden do we appear to have some sort of epidemic issue of bed bugs in america I worked for a local College for 30 years and didn’t have issues until 2 yrs back?
You CAN do this…and good for you…for looking for information.
A couple of bits of advice…
if your insurance covers it…find either a pediatric pulmonologist or an Asthma specialist (these are usually allergy docs, but they don’t only deal with allergic asthma) Pediatricians are GREAT for stomach bugs and strep throat and well checks…they are not always so good for Asthma…I feel MUCH more confident in my daughters care under her pulmonologist…in fact, her pediatrician only sees her for asthma under duress (like if the pulmo can’t get her in at all)
Don’t be afraid to ask LOTS of questions. There really is no such thing as a stupid question….Doctors (even fabulous doctors) forget that they see this stuff every day…but it’s new to you. If you don’t understand something the first time…ask them to repeat it. If they make you feel badly or stupid for asking questions…find a new doctor.
It’s a crappy hand to be dealt, but it’s a totally playable one. Get some good supports in place, and you’ll be good. Even my daughter, who I thought would NEVER EVER be under control is beginning to find some balance of control…BUT, her lack of control comes from underlying issues that are NOT common…Asthma, on a whole, is typically a VERY VERY controllable disease.
This sounds silly…but it has really helped me…I got a pocket calendar, and every time my daughter needs her rescue medication…I write it down, and anything that I can think of that might have triggered it. Identifying triggers is VERY key. It also will help when the doctor asks…”how often does she need her rescue medication” For me…they all run into each other…so being able to pull it out and say “ok…she had a long flare here…and needed it here…etc” really has helped a lot.
Finally…take time to peruse THIS site…Amy has a TON of great information here. She also has a ton of great commenters who really really know their stuff.

My son was also diagnosed at 4. I felt like it took forever to get his asthma under control, but looking back, it was probably only a year. It took that long because I got so freaked out by the thought of giving him a medicine with a black box warning — Advair. So we used Flovent instead and it just didn’t cut it. Once we finally saw an asthma specialist and not a regular pediatrician who could really assure me that Advair wasn’t going to kill him, I relaxed and agreed. From that moment on, things were amazingly better.
I’m not suggesting your daughter needs Advair, just that it may take awhile to find the right medications for her needs. The right drugs and figuring out the triggers got us to a much better place. My son went from being moderate persistent to mild persistent in three years (he’s 7.5 now). And he hasn’t had to take prednisone in almost a year!! It does get better. Good luck!

The one good thing about food allergies is having to read all the labels on all foods. There have been many items I’ve passed on, not becuase it had the allergen in it but because I could not imagine feeding it to my kid (or anyone) after seeing what was really in it!

OK…so ibuprofen is linked to asthma exacerbations (at least in adults-I’m assuming the same is true for children…studied or not), NOW, acetaminophen is linked to wheezing…Aspirin is linked to Reye Syndrome…what CAN we give our kids for pain relief? I had switched to Tylenol almost exclusively, because of the like between asthma and ibuprofen…It would really be nice to have a safe pain reliever/fever reducer.
I appreciated the quote in the Walmart piece, “No family should have to choose between food that is healthier for them and food they can afford,” That’s often what I feel like I’m doing. I can buy whole grain pasta, cheese, flour and butter to make mac and cheese…or I can get a box of Kraft Dinner for a buck and change, that I mix with margarine that I got for 99 cents for a box of 4 sticks. I can get canned veggies for cents on the dollar or fresh veggies, that cost SO much more. I generally compromise with frozen. Very lean ground beef is double the cost of the fatty variety.

The school nurse has to approve anything that a child brings in for snack…and one of the kiddos brought in cupcakes…and the nurse commented…there’s barely anything REAL in here. (but of course, the mom wasn’t allowed to MAKE anything to bring in…which I appreciate, because of allergies)
I tend to pass on anything that I have a hard time pronouncing.
You know, the girls’ school right now has no restrictions on homemade treats. Weird, huh? This is AG’s fourth school, and the only one, ever, that’s allowed parents to make and bring in stuff.
Nothing! They get NOTHING for pain! Just kidding. At some point, and hopefully sooner rather than later, I’m hoping the medical community will consolidate all the current and future research into some sort of recommendation. It’s like we have just enough information to be worried, but not enough yet to do anything effective.
Im late i know, but my daughter was dx’ed at 3. shes 5 now, an we have fairly good control, took about 1 1/2-2 years of fiddling but every step along the way was a little better. Im feeling better enough about her control that she *gulp* starts a 6 week youth basketball program next month, practice once a week and games on saturdays

I’m good with them suffering…my kids often refuse pain relief anyway…thank goodness A can swallow pills, she can take one advil…she hates the kids stuff. M isn’t heavy enough for the pill yet…and she gags on kids medicine…so she often goes without, unless I FORCE her (for a high fever, or whatever)
I agree…we have just enough info to freak us out…and not enough to make a real and true informed decision.
Our district is “peanut aware” which means they don’t serve anything with peanuts, and there are no homemade treats…but kids aren’t restricted from bringing peanut products for lunch. They call it a wellness policy…but it’s not…it’s an allergy policy…which I’m fine with, but call it what it is…if it were a true wellness policy, I wouldn’t be able to send in Oreos…or PURCHASE cookies from the school cafeteria. (of course, no one thinks of ABBY’S allergy…since they have “carnival cookies” which are sugar cookies with either m&m type things, or colored sprinkles…don’t know which…but either way…Abby can’t eat them.)
Funny you should ask that. I have a heart defect, and my asthma doc. recently had me try Dulera…I’ve been on advair for years, take half a dose, and have 6 months of back up inhalers. So I went back on Advair. Personally, I couldn’t really tell much difference, and the fomoterol part of Dulera always made me nervous anyways.
From what I’ve heard, supposedly formoterol is a “better” LABA than salmeterol [in Advair]. However, Pulmicort is apparently a weaker steroid [what's in Symbicort now], so perhaps they’re trying different mixes for different people [which I am SO in favour of, considering I seem to be one of those people who needs a fair amount of Pulmicort with my formoterol to pull me into some semblance of control!
FloVent and QVar are supposed to be the “stronger” inhaled steroids, I think. So the new mix and matching is rather exciting — though I don’t think Dulera has made it to Canada yet.
Oh I completely missed that possibility of matching patients to meds, Kerri. Great point! LABAs are one of those topics that I just don’t have any firsthand knowledge of since AG’s lucky enough not to need them. Thanks!
I’ve recently moved from seretide (advair, by its European name) to separate LABA (formoterol) and steroid (ciclesonide) inhalers, and I have to say that I’m personally very happy with that move.
For me, I like the flexibility of knowing that if I have to change dosages, as happened a few times whilst on seretide, that it doesn’t require getting a prescription for a different inhaler, as it did in the past. It also gives the flexibility to change components independently, to find the best match for each patient personally.
Whilst I don’t have a flexible plan right now that allows for stepping up/down components, I like the idea that maybe that will be possible in the future.
As an adult, I appreciate the flexibility here a lot, though I can imagine that with a child compliance might be more of an issue with taking an extra inhaler.
Sara C, what you mentioned reminds me of when I was in elementary school and they pushed healthy eating… while the “healthy” choices in the cafeteria were 1) nearly twice as expensive as the “unhealthy” food, and 2) absolutely revolting. Oversalted, dried out, burned, and usually serving all manner of kid-unfriendly foods like liver and brussel sprouts. I always thought it was very hypocritical of the school to charge so much for such bad healthy food on one hand while pushing healthy foods with the other.
At my high school, they flirted with the idea of banning all nut products. Interestingly, it was one of the extremely peanut-allergic kids that convinced the school to institute a allergen-isolation and education policy instead (a certain number of tables where peanuts, nuts, and other foods some of the student body was allergic to – such as strawberries one year – were allowed to be eaten, well away from other sources of food, which were cleaned after everything else in the cafeteria. The school didn’t prepare anything with nuts in it, but packages of nuts and/or peanuts could be bought from the vending machines. My school was small, used disposable wipes for wiping down surfaces, and had a paid cafeteria supervisor to enforce the food isolation policy, so it worked fairly well). This student, with the support of her parents told the administration that isolating her entirely from peanuts, wasn’t going to help her, as in three years, she would be off at college and expected to manage her allergy on her own. She said she would be better served by educating the other students and giving her the tools she needed to manage her own food needs.
I have many issues with my high school, but I approve of how they handled their food allergy policy. Education and giving the affected students ownership of their own health, with appropriate safeguards and education, is probably a better overall approach, at least for high school kids, who are old enough that they really should be taking charge of their health to a degree, anyway.
Yes since the FDA changed the Proventil inhalers to HFA, My asthma is no longer under control. That’s all I ever needed and now it doesn’t help at all. So now my Doctor has me doing Prednisone, Symbicort, Dulera, Nasonex and I also have the breathing machine for Albuterol Sulfate. Also I now have to go to a lung doctor. All these other drugs lead to death, it says it right on the lables, except for the Albuterol Sulfate. They say they took them off the market because of the ozone…..This is a bunch of crap they took them off so they could make more money off of all the other meds.
@ Rena Actually, the FDA and drug companies had little to do with the decision to remove CFC inhalers from the market. The push for that was the EPA, which mandated with the Clean Air Act that the CFC inhalers be phased out as soon as alternatives were available.
Regarding Dulera and separate LABA/steroid inhalers… Advair works fine for me, when I’m not flaring, and then I have to add steroid, not subtract it. I haven’t gotten lower that 500mcg BID of fluticasone in nearly two years (and even when I was lower, I shouldn’t have been) so it doesn’t bug me. If my asthma ever lessens enough that stepping down the steroid is a realistic option, I’ll speak to my doctor about it. As for Dulera, I might try it, if my doctor brings it up, but I’m relatively stable now and I’m reluctant to mess with what seems to be working.

-Interesting article, but…I live in the NW, home of really high rates of autoimmune diseases such as Lupus and MS…as in some of the highest rates in the country. I don’t think we’re that clean up here, but that the theorized link between very low vitamin D is likely; however, these are much higher in women, which I suspect has more to do with estrogen and all of the fun hormonal differences between men and women. I must concede that my asthma is very mild and I was a full-on tomboy in the dirt and sand, so….maybe?
-I love any news about more effective treatment on the horizon. It gives me hope.

Regarding the hygiene hypothesis and girls being “cleaner,” I’m unsure.
Here I am again. We made it a little over 2 months. My little guy has had a cold (maybe allergies) for the past 5 days and tonight is cough turned on us. I told my husband at dinner that I thought this was going to be a rough night. Sure enough. I woke lil guy up and gave him benadryl, blew his nose and gave him water. That was like winning a minor war- let’s hope I don’t have to neb tonight as I don’t know how that will play out. Now I wait (DH is sound asleep. Why is it that mom’s are usually the only ones who lose sleep?)
In the past few months we’ve instituted heavy duty dusting and vacuuming. I invested in a Dyson Animal and love it.
I am sitting on the couch timing his coughing to see if I can get a sense of it worsening or if the benadryl will have a positive effect.

I do what I used to do when I was a kid: If I’m feeling good, I’ll suit up for some sledding if I have a sled (sadly, I don’t at the moment). Otherwise, I curl up with a book and get some reading time.
When I was younger I would always go sledding with the other kids in the neighborhood and make snow forts… made from giant snow balls. We never were big on making snow men, but we figured out how to build igloos and make walls and things out of the snow. Though, I’m not sure if you’re snow there is wet enough to hold together for making anything with it or if it’s a snow better for skiing.
Or, if we didn’t play in the snow, we’d find something inside to do, but it usually involved wishing we were outside in the snow
Believe it or not… in my nearly 17 years of going to school I’ve never had the privilege of a snow day! Sometimes the bus would get us there 3 hours late because of terrible roads, but school has never actually been cancelled for me.
But on weekends when it snows a lot there is tobogganing and baking to be done! When we were kids, we built snow forts. We’ve even used a planter as a mold for an igloo block and built an igloo. Fuuun times.

Canadians can’t watch stuff on Comedy Central (or the American Discovery channel, much to my irritation – I want Storm Chasers, darn it!), but fortunately, the Comedy Network runs the Colbert Report.
But then, as far as I know, you guys can’t get CBC, so fair’s fair, I guess.
The university I attend has been closed all day only three times since I started attending five and a half years ago. Two of those were for blizzards that dumped over a meter (roughly 3′ for my Southern neighbours) of snow each, and the remaining one was for an ice storm that incased everything in literally an inch of ice.
My last elementary school and high school were much more cautious – more than once, they cancelled as a precaution, and we ended up with a gorgeous, sunny “snow day”.
Back when I lived in Manitoba, we never were cancelled for snow since ~95% of the kids and teachers walked to the school. However, we were cancelled on more than one occasion for cold. If it was too cold for the busses to start, and the wind chill was low enough that walkers were risking frostbite and hypothermia even during a five minute walk, school was cancelled. That happened at least three or four times a year.
I really enjoyed that video clip. If you purely look at risks and benefits it makes good logical sense unless you have allergy to vaccine components and/or immunosuppressed. I have missed a few flu seasons due to latex-contaminated vaccine supplies but thank goodness for herd immunity!
I got quite a few Ice days in elementary and high school. However, Ice days are not much fun, since ice day generally correlates with no power. Thus it is a lot of careful heating of the house wearing layers indoors, culminating activity of the day sometimes being backdraining the pipes! Party on! However, on our few snow days Sledding and Igloo building were definitely the activities of choice.
We’ve got a snow day today and yesterday for the 2nd and 3rd time in university history, (38 years since the last one). In northern missouri, we just keep layering up and go to class anyway. I own ice cleats for my boots, and flannel lined jeans for a reason.
ahhhh….we played Wii, and did homework, and cleaned the playroom…on the second day, we fought. Here in New England, we just had our 7th day off due to snow. Each one fully justified…but still. In all my years of schooling, we went over the budgeted snow days one time…and only by one day. My kids are over the allotted days by 2, and it’s only February…there’s a whole lot of winter left. By the seventh day off in a month, my kids have run out of “fun day off things to do” On the first few, it’s fun to go outside and build things…but when you’re not even a full 4 feet tall, and there is 3+ feet of snow on the ground…it’s not so much fin to go out and play in it any more.
BUT…spring will come…and if we’re lucky…we won’t wash away in a flood…(remind me to get the stuff in my basement off the floor) and then summer will come…and we’ll all complain about the heat and humidity…it’s the circle of weather.
Heh, “circle of weather.” No humidity here, but I’m scared of the ragweed this summer after such a low-snow winter. Maybe we’ll make up for it these next couple of months.
They ended up with two days this week. For cold! And barely any snow!
I guess I shouldn’t complain. It was really cold here, with apparently some of the lowest temps this area’s seen in years. And these were their first days off after all.
Universal flu shot is nifty! I hope it’ll be on the market sooner rather than later (but one of my profs told me that drug development can take as long as 15-20 years, so I’m not holding my breath)

We think that cold weather definitely affects our son as well. We moved to the bay area from Seattle and our son definitely does much better here. In Seattle, he would fall sick during fall and winter constantly, but not as much here.
Once we are finely tuned to what is going on in the environment and our locality, we are able to better control his asthma.
my son is only a year old and gas to use the nebulizer. of course hehates it and all of these suggestions seem to be for older kids. anyone have any ideas on how to help? i have to hold him down and he screams. then he screams for a long time after it is over because he is so stressed ot from it. Unfortunately, I don’t. My daughter was misdiagnosed until she was 2 years-old, so when she was your son’s age, ER docs kept sending us home with oral meds rather than a neb. Even as a toddler, though, my daughter screamed for a few months until she got old enough to distract. I hope someone else out there has some ideas for you!
Dulera does NOT work at all for me… I will stick to my Advair 500/50. I’ve been on it ever since it came out and I love it. I prefer the diskus over the HFA. As far as my Rescue inhaler (albuterol) is concerned. I want the old one back. HFA doesn’t work as well. I have tried Pro Air, Proventil and Ventolin. Proventil and Ventolin work better because you dont have to keep priming them if you dont use it for a while. But I say bring the old ones back. Clean up the air pollution and we won’t have as much asthma.

Dulera is very new to me, and I have not come across anyone locally using it. As to the two ingredient, it’s basically Asmanex (Mometasone) and Foradil (Formoterol) mixed together. In the studies I’ve read it seems to be very comparable to Advair. I did read though that the Fluticasone in Advair does have less systemic effects when compared to Mometasone because it seems to be a more receptor specific steroid. But the Formoterol seems to be slightly superior to the Salmeterol in Advair due to it’s faster onset time. The down side is that it seems to have more severe systemic side effects ( the Jitters.). I guess it all boils down to what works best for you. At the very least it gives all of us asthmatics a bigger choice in controller meds.

I just wanted to chime in again about all the controversy surrounding the CFC to HFA conversion. I’ve been told by many patients that the HFA inhalers do not work like their “old” ones did. As for me I really never noticed a difference. The thing I don’t understand is that the medication is still exactly the same in the new inhalers, the only thing that was changed was the propellant. The only difference that I personally noticed is that the HFA inhalers don’t seem to squirt with as much force as the old CFC inhalers did, but that might just be my imagination, who knows.

We started my son on a nebulizer today and he is using the mouth piece, not the mask (He is almost 3). The nurse told me that I could stuff a cotton ball in the back side of the mouth piece (where the steam comes out) and just let the mist run on his nose and mouth if I have a hard time getting him to put it in his mouth; she also said this way works well when they are sleeping. She said not to put in the cotton ball if the piece is in the mouth though.
I wonder if part of the reasoning has to do with this being a cash cow for allergists’ offices? Mine wants to charge $20 per visit for shots – given that it takes about 60 seconds to stick you in the arm (and then you wait in the lobby for 30 minutes), I would say there is a high profit margin on that service!

Hi Debra–My asthma kid has done just fine ever since we moved from FL to CO, and we’ve even taken her hiking as high as 10,000 feet. Every case of asthma is different, and I’d certainly follow your doctor’s advice, of course, since he/she knows your daughter & her health best, but we haven’t noticed any worsening of symptoms after 2 years here. Moving to high altitude WAS a huge adjustment for all of us, though, so if you let her go then please talk to her doctor about preparations and possibly pre-treating.
I have used all inhalers on the market but dulera 200mcg/5mcg is great. 2 puffs in am I can go to the gym I have no breathing problems, then 1-2 puffs in the evening. It is like I have new lungs. My doctor has given me 2 samples and a perscription. My doctor had to contact the Ins. co. to get their ok, but I have never got the prescriptions filled. The cost is too high. The doctor gave me a discount card but he didn’t read it. The card said if you are on medicare or medicare part D, the discount card would be void. So even though Dulera really helps the cost is so high, and on a fixed income we can’t afford it.
Dulera hasn’t been on the market for terribly long, so the actual safety and adverse events reported to the FDA may take a while to get a handle on.
The component of these medications that has been most worrysome and that carries the black box warning is the long acting beta agonist (LABA)
In Advair, it’s the drug salmeterol
In Dulera, it’s formoterol

While the main study that led to the black box warning was the SMART study – which looked specifically at salmeterol (found in Advair), the FDA extended the warning to the other products in the class.
There have never been any significant clinical data comparing the safety of the two products head-to-head.

we are going to move to Colorado for my fiance to finish his school program. We have children too. I have asthma and I’m glad to hear that I wont be affected much. I was worried about the higher altitude and getting dizzy. Did you notice anything like dizziness? It sounds like such a great place to live. Everyone I talk to say they’ve loved their move. Is it really sunny? We’re in Ohio and its’ always gray. Rains too much, snows too much, and not enough sun. Not to mention a horrible economy. People generally seem happier that I’ve talked to in terms of being happy with their quality of life in Colorado. I’ve never met someone who just “wowed” over Ohio. And I’ve lived here my whole life. Thanks for posting this blog! It gives me more things to think about. Now I have to pray I get a job. I hope its not really difficult considering my Ohio address on my resume. How is the economy right now?

This is a great blog on asthma. I have allergies and certain things in my environment were triggering my asthma. Unbeknownst to me, the house I was living in had mold. The doctor put in on Advair which has a steroid in it and I took it for the whole 5 years I was living in the house. Once I moved away from the house, I noticed I not longer experienced dilibitatin asthma attacks, but what I did notice is that my body was still reacting to chest tightening when the advair wore off. After doing much reasearch, I found that I can’t just stop taking a steroid without experiencing a horrific asthma attack even though I was not longer exposed to the allergins that initially triggered my asthma. It took me almost 2 months to get myself off the Advair by slowy reducing the number of times I took. I was finally able to get off it completely. While I believe these meds are necessary for some people who suffer from COPD, I am concerned that these steroids are making people more dependent on them due to the steroid affect on the body, instead of working with an allergist to help build up immunities. I never will take a steroid again I rather try to treat my allergies alternatively. I now have side effects from taking it for so many years and have a catarack in m right eye, I’ve experienced a mild stroke, and some decreased bone density. I really think people need to know how these steroids adversely affect their asthama long-term.
Inhaled steroids, such as advair, are inhaled and exhaled, and therefore do not stay in the body and cause debilating side effects, such as brittle bones, cataracts, thinning of the skin, etc. However, oral steroids such as prednisone, taken over a long period of time can. I was prednisone dependent for almost twenty years and went from 135 lbs. to almost 300lbs., cataracts in both eyes, depression, and bone degeneration, and severe mood swings. After an extended hospital stay (one of many), I found a wonderful pulmonary doctor who was able to finally get me off the oral steroids. But the physical damage was done. I now take advair, singulair, albuterol, and an inhaled nasal steroid and am doing much better. I do have breakthroughs from time to time and my doctor will put me on an oral steroid dosepak that I taper down on. I can only hope that I never have to go back on daily steroids. I was much younger then and don’t think my body could handle it now.
I just got switched from Advair to Dulera. All my research has led to me to find that they are equally as safe (or not safe as you might look at it). There seems to be a small variance in which side effects are more commonly seen, but those more severe side effects only seem to be an issue for certain particular pre-existing conditions. Advair was just not working, which is why I switched. The following paragraph can point as to why for me my doctor and I both think Dulera might be a better option.
Advair is a larger particle size and travels slower and a shorter distance. This prevents it from absorbing as much into the smaller airpassages deep in the lungs. Dulera is a smaller particle which is also more evenly dispersed, hence has greater absorption in the lower parts of the lungs. This explains why in patients like myself, where the lower lungs are the issue, it has been claimed to be more effective. We’ll see, but I’m hopeful.
As for HFA, the comments about the speed are right. HFA inhalers have a much lower speed when they exit the inhaler. This is actually better for inhalation as it also causes less chaotic airstreams, thus more medication actually reaching the lungs. HFA does not have the components in it however that can give you that “high” effect. I noticed a big difference when my proventil switched. I also noticed though that the allergen counts had been shifting higher as well as other pollutant counts. In addition, I found that my new proventil really only cut it with the spacing chamber. (on a side note look for the widest chamber you can find, it’s the width not distance that had a greater effect on the amount of medication lost in transit) Also, shake your new inhaler more vigorously than you might have the old CFC ones.
Oh, with both Dulera and Advair you can get thrush. (they have steroids in them afterall) If your asthma or something else is compromising your breathing though, Advair will cause it more so. It has no propellant to aid the medication in reaching your lungs and not settling in your mouth/throat. And as I mentioned above, it’s larger, hence slower, particles.
I hope this helps someone! Often I find knowing even a glimpse of some of the science behind a medication can help you decide to switch,start, etc and not leave you relying on your doctors experience or even another patient who’s cause might not realistically reflect your own.
Hello, it’s nice to know that I am not the only one that is trying to help her asthmatic child. My daughter is 2and a half and she has allergies to cats, dogs, dust mites, mold, shell fish, she also has exzema since birth which has spread to her hands, ankles, tows and fingers, she is asthmatic because when she gets a cold I am instructed to use the orange and blue puffers. Last summer I didn’t need to use any puffers but this year it seems as things have gone for the worst, she is allergic to more things, her excema has spread to other past of her little body and I am scared not to use the puffers because I am scared that she will end up in the hospital. I keep questioning my instinct do I give her the ventilin, does she need it. I don’t like giving her the ventilin puffer (blue puffer) it makes her very shaky for a couple of hours and what’s strange is recently I noticed that after giving her the blue puffer her face turned all red? What does that mean, is this an indication of something? My pediatrician didn’t answer on this. Then I keep listening to her and She seems like she is sometimes breathing faster and short? Does this mean she is having a attack? If I don’t do anything will this go away on it’s own? Some times she seems like she is breathing really deep and not rapid like it’s hard for her to take a breath and she really needs to inhale? What does that mean…..her oxygen level is lower? How do I detect this? Is there a device that you Moms heard of to detect oxygen levels, I am so scared because at one time when we were in the hospital she needed oxygen and that’s all I see, can’t get this out of my mind, it’s killing me not knowing what to do at times. any suggestions? After years of fighting through tough asthma episodes and various drugs, my allergist and I spent the better part of a year trying various drugs, combinations and doses, and finally came up with using a combination of Advair (250) and Singulair. Those two together have kept me completely under control for over 13 years now, with almost zero need to use albuterol, other than a rare couple puffs a year. If I was ever off of either drug for more than 3-4 days, I could feel myself getting tight, but together they did the job.
The problem has been that after so many years of corticosteroid use, I was developing a constant dry cough/clearing daily. It had gotten to the point where I was tired of having to explain to friends and coworkers that I was OK, although they were telling me I was disturbing everyone around me non-stop.
My doc has decided to try a switch to Dulera, and I’m on a 12-day taper of Prednisone to clear things up. Today was day 1 of Dulera. I’ll look forward to hearing any other advice or experiences that any of you can share.

I have had allergy for years and this year the tree pollens in nyc area got so bad that I started coughing pretty badly. Half weeks into the cough I began to experience wheezing and shortness of breath, chest tightness. sure enough my allergist believes its likely allergy induced asthma. I started taking Dulera last night (2 puffs) before i went to sleep, 2 more puffs this morning, its now 10:25am and my symtoms: cough has gotten somewhat worse/frequent, chest tightness somewhat worse.
I really hope dulera will work. Not sure what i’m gonna do if it doesn’t

I’ve been on Dulera for about a month. I’ve had asthma for almost 40 years. I have not tried advair before, or any other LABA or combo inhaler. Must say that it does work for me. My lung cap was 70% prior to starting, so my asthma was not as bad as some people’s, although it worsens periodically during the day. I am on half the recommended dose of the 100/5 inhaler (2 puffs/day) and did notice some minor side effects the first week or two, mainly a slight headache and some chest tightness right after the inhalation. With regular use, however, those effects have now disappeared. I still must use my HFA inhaler (I clean it regularly!) maybe once a week but it is not daily use as before.
I was very concerned with the warnings – noted by others – on the Rx info sheet however those are the same for other long-acting LABA inhalers. The “death” risk with combo inhalers like Dulera (LABA/steriod) is supposed to be much less, however my personal feeling is the FDA allowed the consumer to be used to test that clinical observation rather than requiring more pre-market testing. My Dr. did tell me that the labeling is there only because it contains a LABA, and that the combo is safe. He also said that the LABA of concern with the death risk was not formoterol (in Dulera), but salmeterol and was of concern only when used solo, without the steriod addition.

I have COPD. I used Advair 250/50 from Oct. 2010-April 2011. I was still getting SOB and using oxygen. I was changed to Dulrea 100 in the middle of May. This medication is good(for me). I am now walking with little SOB don’t use my oxygen as much. On June 4th, was my second outing. I was able to walk a longer distance, but once I started walking up hill I havd to use oxygen @ 2Lpm, not the 4Lpm that I normally would use to walk when out. It was ok! I could feel better lung expansion.
I could not do any of this using Advair.

I have been on Dulera 200/5 now for about 4 months in an attempt to get my asthma under control after getting repeated episodes of thrush from Advair. I really can’t see a difference in the two but agree i tend to get a little relief much quicker with Dulera than i did with Advair.
I’ve had asthma all my life but only began treatment for it in 2000. I was given Singular and Proventil and WOW…was I a new person, joined a gym and lost nearly 100lbs. Just in 2008 my symptoms began worsening and I was put on Advair, worked great for me for a while…until repeated thrush (even with rinsing as directed). Singular was a miracle drug for me!
Yesterday I saw a new Pulmanologist….he is doing all the pulmonary function tests (which i’ve never had…i know..i couldn’t believe it either), blood test to check for allergic asthma, etc. One thing he did give me to try was an rx of Prilosec 40mg 1x daily an hour before evening meal. I took my first one last night and…well….this morning I felt…so…. good. I wasn’t all congested as usual and had lots more energy. I did a little research on GERD and Asthma and found that 80% of individuals with Asthma have GERD and that works visa/versa as well. So…I’m really hoping that this little improvement i’m feeling is going to continue…I’ve been to the point of taking my rescue inhaler 2-3 times a day.
So, as far as Dulera….I haven’t seen a big improvement over Advair but i do feel better 15-30 mins after taking it with easier breathing. It just does not last the 12 hours i need it too.

The pharmisist tried to give me Pro Air, even though my prescription said ” Ventolin” telling me it was the same. Its not! Its double the money of Ventolin and only works half as well. I scolded him for attempting to push something on me that was not on the prescription. To hell with the profit margins of these drug companys!
What I know about flu then is nothing compared to the flu at present. The strains are different and more stronger so people tend to be sick longer and requires stronger medications. I remember the older days when 3 days is the max in bed, then get well without seeing a doctor, plus medicate with plain bed rest and chicken soup.

I really think that the better bronchodilator is the difference here.

My doctor says I must exercise so I tried walking with my daughter-in-law but couldn’t continue because my arthritic knees hurt so badly. A few of my friends said try swimming it takes the stress off of your joints so I joined our local indoor pool this spring but began having bronchospasms and trouble breathing in July. I have NEVER had any breathing or coughing issues before in my life. My husband said maybe it is the chlorine as each time I go swimming now I come home coughing, wheezing and having to use an inhaler. He suggested I do some research. What I have learned is really disgusting and I am now afraid to go back in the pool after what I have found out.
I am actually on the internet right now searching for a non-chlorinated pool but have not been able to find one in our area – central New Jersey. I am so bummed out. The swimming seemed to be a life-saver for me – I was feeling better, losing weight and my blood sugars were so much more stable. I have spent hours and hours on this computer researching the effects of chlorine and available alternatives to its use in swimming pools with no luck. Help please!

I was on Advair 500/50 & Albuterol for years & was still wheezing during the night during bad allergy seasons. I started Dulera in February & have not had any wheezing since. I still have my allergies, but without the wheezing. I started out at 2 puffs 2x per day, but now I only use it once a day. My doctor (Head Asthma doctor at Kaiser) told me they were having great success with Dulera & thought I would be a good candidate & would be able to cut down how many times I use it, as well. I am so glad that I changed to Dulera. It is cheaper in the long run, as I don’t have to use it twice a day, unless needed & most importantly it works for me.

I’ve been using dulera for five months- loving it. I was on advair for over ten years. It became less and less effective during air pollution weeks etc. my symptoms included coughing, coughing when laughing, unable to sing or laugh without coughing. During allergies or a cold, uncontrollable coughing. Advair with a rescue inhaler just completely failed to handle it. I changed from allegra with advair and flonase, to astepro (as needed) antihistimine, singulair nightly, and dulera every twelve hours and flonase. The difference is unbelievable. I have my singing voice back. I can laugh. I can talk without coughing. I can walk a couple miles until I run out of time- not breath. Huge difference. No more rescue inhalers getting used up or used at all. thank you thank you thank you- dulera.
when I was hiking the first summer I started advair, I noticed it opened my lungs to 800 on a peak flow, then dropped bam- while walking on a trail to 200, then reopened, etc. advair was the best they had- but using it was a freaky experience with regard to excersizing. I am not experiencing this with dulera- I just am- steady- and it doesn’t run out like advair – I was waking up at 4 am with advair, too soon to take the next dose having difficult time breathing. Now with dulera- I can simply breathe & be-
why is it so much more effective for me? could it be the size of the particles or the method – its a puffer not a packet you breath in- initially especially with irritated and inflammation due to allergies and air pollution in addition to whatever lung issues inherent to the situation– it just went past the inflammation and deeper into lung tissue- without breath in being an issue- it flew in by itself basically all I have to do is aim and shoot while I breath.
I am so relieved.
Initially I used prendisone and z packs, then kenalog shots when I could not longer breath (which of course wore off.) I started with asthma cort & a separate seravent- which I liked, but everyone went to advair back then…. changing to singulair (smallest dose 10 something) helps-
but for three months I used dulera alone with antihistime and no stingulair or oral steroid, or steroid shot- and the dulera stood alone changing my breathing. Its just different from advair. I think its about inflammation and advair not competing well with the particulate matter pollution etc. in the air- it failed. It became ineffective. I’m in for dulera.
I think I can keep working with the career- I have a new voice for work. Its steady- its strong, I can breath all day long & I can keep my job. I’d been wondering about it.

My doctor recently put me on dulera. I haven’t noticed any positive chg from the advair. However, he also put me on Combivent for my rescue inhaler. This medication worked wonders for me. It has the albuterol like the old inhalers (only till 2013) had as well as another medication.

I recently changed to dulera,singular,nasonax and clarinex d and I have to take 2 allergy shots once a week .I have ventolin as a rescuer and I have albuterol sulfate to take in my plug inhaler. And so far everything has been fine. But like someone else mention I guess it all depends on the person.

It’s good to hear about everyone’s experiences with asthma. Just today my dr. switched me over to Dulera from QVAR because I’ve been having flare ups over the past 3 months. I’ve had allergies all my life and adult onset of asthma in 1995 which was also around the same time I was diagnosed with GERD. I’m on an 8 week trial of Prilosec (to replace Ranitidine). I also eat a gluten free, dairy free diet because my heartburn would flare with gluten and dairy. I’m really resisting cutting out coffee because I love coffee (even decaf), but I think it will help the GERD which in turn will help the asthma. I don’t drink wine or beer because the sulfites trigger wheezing. I miss a cold hefeweizen in the hot summer with a slice of orange . . . oh well!

I have had severe asthma since I was a young child (now 48) I switched to Dulera 14 months ago and have significant improvement in my asthma symptoms. The only down side is a find that by 10 hours intomy dose I can feel it wanning and my symptoms starting to return. I take 2 puffs twice a day. I also use generic uniphyl. I do get thrush alot even though I gargled and brush my tongue after use. Itmay be that my Lupus makes it easier to develop thrush. I am very pleased with the meds!!
Great post! Love the feedback. So I just got switched over from my usual asthma+allergy meds. from 2008 till middle of last month. Qvar 40mcg, singulair 10 mg, and pro air hfa to dulera 2 puffs twice a day but I got to keep the other two. But I’ve noticed that my normally low immune system took a turn for the worse since starting dulera. I’ve gotten conjunctivitis 4 times in the last 5 weeks already but what worried me the most was last sunday I had a high fever, pressure in the inner ear (which affected both ears), chills, body aches, nausea, dizziness, nasal congestion, 4th case of conjunctivitis and sore throat an hour after my mom coughed nearby. So now I HAVE to take prednisone to fight of a viral immune virus in my own body (my doc said that virus is not contagious, it is my body attacking itself, but that the conjunctivitis is and that it’s going around this time of year). I know that that’s a side effect and everything but does anyone else have similar problems?

Stumbled across this site after googling ‘advair vs. dulera’. I’ve had asthma since I was 10 mos. old (now 40), so I have been through all the trial-and-error of new meds. After my first child was born 20 months ago, my asthma became out-of-control for the first time in 18 years. I’ve been on Advair 500/50 (one puff AM and PM), but my insurance company will no longer pay for it, switching all Advair patients to Dulera.
My concern after reading the above messages is the jittery side effect of Dulera. I have a history of panic disorder, exacerbated by the asthma, and am a little nervous about switching over. Any other side effects or changes that people have noted after making the switch from Advair to Dulera? Thanks for a great blog!

I have a muscle disease. Consequently, my lungs are affected (decreased function). I’ve used Advair but had to use another inhaler with it. Dulera seems to be stronger and more effective for me. I have not experienced any side effects to my knowledge.
I just read the side effects with my daughter, and we are shocked to know that Dulera caused spinal bone reduction in 85%-88% of all patients using it. In children it causes growth reduction.
Find something homeopathic, I will, I know it is there. Anyone who takes this regularly, will have some very serious side effects with bone breakage. Pregnant women will give birth to kids with a cleft palette, it caused it in mice and rats. Mice, rats and rabbits could not reproduce after being given Dulera. Pharmaceuticals are never the answer.
We have to look into our lives and what we can do to reduce the allergens that cause Asthma and /or worsen it.
Mrs. Pass gargle with salt water. It works for both symptoms I suffer from when my sinuses are a mess, the drainage down my throat and dryness because I can’t breathe through my nose all the time. Same concept with the nasal irrigation but for your throat

Another possible sign of asthma in infants is developing RSV a couple of times in a few months. My youngest got it 3 times in 5 months from January to may. Now that she is 2 years old, her asthma flares up in September, around thanksgiving or Christmas and in march for sure. I have been using Advair and it didn’t help my symptoms last week. Had to go to the emergency room because the wheezing wouldn’t stop I have been using Dulera for two days and it has helped me. The side effects of Dulera are fat less than side effects of Advair. They all have side effects but my goal is to be on this short term.
I used Advair for 5-6 years and it gave me chronic laryngitis. I had to stop using it. I have been using Dulera for a month and it works very well for me. I am a little concerned that it will bother my glaucoma. I will get regular check ups and keep an eye on that but so far so good.

However, I am a pharmacy student, and I would like you to know that asthma specialists do not have these concerns and feel that long acting beta agonists used in combination with an inhaled corticosteroid are fine. Growth in children only slows, and they may be a centimeter shorter than normal, but they will catch up eventually. Cleft palate is a possibility during birth, but the risks of having an acute asthma exacerbation while pregnant are much greater as this can cause harm to the fetus as they don’t receive enough oxygen. There are risks with every medication, but with the help of your doctor and pharmacist, you can alleviate those risks and get the most from your therapy. Avoidance of allergens is always a good thing to do, but some are unavoidable for some people, and we have to do the best we can.

I just started using Dulera (never used Advair) a few weeks ago for my Asthma. With the first time (of the higher dosage) I had a rotten headache, terrible shakiness and then my head felt really foggy and heavy. I called the pharmicist and he recommended waiting a few days and using the lower dose. I experienced the same reaction. Is there anything else that I can take that would not cause these reactions. (I have just been diagnosed with Asthma at 53, so this is all new to me).

Interesting subject. My new asthma doctor said he thinks most asthmatics hate Advair — the medicine that made my hardluck asthma go away. He wanted me to try Symbicort or Dulera. He gave me a free Dulera. I tried it for a month. He said Symbicort and Dulera are the same because they both contain formoteral (a LABA). It was nice that the medicine worked fast to end an acute asthma episode, much faster than Advair. This was true, and nice. However, the side effects of Dulera were stunning. My heart raced after two puffs and I was jittery all day. This made me decide Advair was better for me. Advair never made me jittery and nervous and never increased my heart rate and force. And Advair gave me equal control over my asthma than Dulera. Advair is better. And that’s my opinion based on my personal experience.
Allow me to add, however, that medicines work different for each patient. So all medicine should be available, and it should be up to the patient and the doctor to decide which one works best to control their asthma.

I used advair for years and it really helped me. Dulera has not helped me much, still have asthma issues while on it and it has cause a slew of side effects: dizziness, heavy filling in head, anxiety, nervousness, panic attacks and throat issues. Not to mention muscle weakness, etc. I am switching back over to Advair asap. I gave Dulera two months, so it is not like I am making a rash decision. Every patient is different, so it is always going to be trial and error with everyone. Use what works best for YOU.

Re:the bone density study, you did not include all the info from the study, in the 2 year double blind study of 103 asthma patients treatment with 200mcg twice daily resulted in significant reduction of Lumbar spine bone mass density compared to placebo, and with long term use of inhaled corticosteroids decreases in bone density have been observed. But from my stand point as have asthma for 37
years and also been a certified licensed Pharmacy Technician and haven taken everything on the market, Advair has worked the best for me without side effects, the Dulera has not. But Dulera made a better deal with most insurance companies to be a little cheaper so the insurance companies are refusing to cover Advair even if it works better they will not give a formulary exception unless Dulera causes allergic reactions or a determent to your health. So after they have to pay for your expensive hospital stay then your ADVAIR WILL BE COVERED.

I have had severe asthma for at least seven year. About a year ago, I started using Advair and finally got relief. I love it and have not had a single full on attack since I started it. Believe it or not, I only had to use one spray every other night and still feel great! What I already don’t like about dulera is it says to take two puffs morning and night! ?? Who wants the extra work when the advair was so easy! Plus, on the two nights I tried it, I could not sleep! However, I am nocturnal to begin with but I was up until 4 am feeling anxious. This can cause other health issues – exhaustion for one, haha. Seriously though, I didn’t even think of it being due to dulero until readung the other replies. Thank you!

I had a recurrence of asthma during the fires and smoke in So Calif 2006. Plus a cat moved in. I was on Asmanex (corticosteroid) for over 3 years. Due to the price I only used it half the time I should’ve. Then a new doctor put me on Advair. At first it was great, never needed the rescue inhaler. THen it started not working as well. Now my HMO switched me to Dulera without any notice. The development of osteoporosis is common to all 3 meds. I’m 66 and of course have it, but part is family history as well as taking Prilosec (for ulcers) which has the same side effect. I read all the side effects of Dulera, and have the same concerns you all did. I haven’t started it yet. One day left on the Advair. Thanks for your input. I know now what to watch for.

I have started to take Dulera and for me. I can now walk across the room without losing my breath, I can sweep the kitchen floor and take long walks now. With Advair I could not do any of these things,

I had been on a Advair for a year when my HMO switched me to Dulera for “pharmaco-economic” reasons, i.e., they are getting Dulera cheaper. After just 2 weeks on Dulera, I am getting night (and sometimes day) dry coughs, which usually preceed an asthma attack. As of today, I am back on Advair. It’s hard to believe that the 2 drugs act so differently for me, but I am hoping they do (or else I will have to further investigate this cough).
My 4 year old’s behavior becomes outrageous right before a flare. She will bite, kick, pinch other kids and runs away from me so fast so I can ‘t listen to her wheeze. Sure enough if I can get close enough to see and listen, I hear her straining and see the pulling in her throat. I’ve worked all year on teaching her to explain what it feels like rather than acting out.
I just find sleeping time to be the only sure time to get it in them right. If necessary while they are awake, of course watching tv or laptop but they can’t really hear it over the awful hum. We’ve recently let our little one use headphones during this time to keep her occupied. And don’t think I haven’t allowed her to do anything she asked to get it done, cutting with scissors, tearing through my jewelry box or anything else that was previously on the forbidden list.
I have tried many preventative inhalers including Advair. None of them ever seemed to work. I have been on Dulera for 2 months now and have not had to use my rescue inhaler for about the last 8 weeks. I am very active and before Dulera would have to use my resuce inhaler basically everyday to work out.
I have had no side effects from Dulera. My Doctor recommended washing my mouth out prior to every use to avoid any possible throat issues.
The device is pulse oxymeter or sleep study pulse oxymeter and buy from eBay or amazon to meaure u2 level at home.
