A twenty year survivor of transplantation has developed swelling of feet, diarrhea, fever... this could be symptomatic of what?

The patient received a heart transplant at 9 days of age. Underwent a kidney transplant at 9 years, after drugs caused kidney failure. Now, at age 20, he is experiencing severe swelling of feet, has had diarrhea for 3 weeks, and had a fever (102) recently. He has undergone blood testing and echo exams with nothing critical showing up. Any thoughts on what could be happening here?

    Has he pain feet?has he ever had varicose veins or tromboflebitis? Has he had an echocardiogram with doppler? Tromboflebitis, for example, could have this symptoms, but is very difficult do a diagnosis without a clinical examination and medical test. I would like to recive news.
