    The blood pressure given is not possible - the top number is, almost without exception, always higher than the bottom number. That being said, blood pressure control in patients with Alzheimer's disease is no different from blood pressure control in any other person. When deciding which medication to use, it is useful to look at other medical issues that may receive benefit from a specific medication. For example, a patient with diabetes could receive an ACE inhibitor such as lisinopril. A patient with Raynaud's could use a calcium channel blocker such as amlodipine.

    People with blood pressures significantly elevated frequently require multiple medications to help control blood pressure. This may require frequent blood pressure monitoring to titrate the medications without lowering pressure by too much.

    Of course, I fully agree with Dr. Touhey and the comments about blood pressure control. With regard to the presented figures, if the Question was written hurriedly the writer may have been attempting to write 120/90, though if so a normal blood pressure would be less than 120/80, therefore 120/90 would represent "pre-hypertension". For this blood pressure, repeated readings at other times should be performed to establish the this is the average blood pressure. Some blood pressures are recorded higher in the doctor's office, so-called "white coat hypertension" but may be normal if taken by the patient at home. At this level 120/90, patients may benefit from weight loss if overweight and reducing salt intake if excessively consumed.

    The other possibility was that the writer of the question misunderstood which number was systolic (upper number) and which was the diastolic (lower number) and that instead of 120/190, the blood pressure reading was actually 190/120. This would represent High Blood Pressure Stage 2, a potentially most serious level of blood pressure. In this case, if found to be consistently elevated to this degree, prompt drug therapy would be indicated with careful blood pressure follow-up. If the patient doesn't respond to multi-drug therapy attention should be given to specific medical causes for the high blood pressure in contrast to the more common "essential hypertension" which most patients with hypertension have and no specific pathologic conditions are found which cause the elevated blood pressure. ..Maurice.
