What is the best way to determine if someone has a sleep disorder?

For at least 6 months I have not been able to go to sleep. I will go to bed around 11pm at night and not be able to fall asleep until 2-4am the next morning. I have a regular sleeping schedule and sleep about 8 hours every day, but it is just so difficult to fall asleep. I also usually wake up with morning headaches, which sometimes lead to terrible migraines. During the day I am groggy and feel as though I did not get enough sleep, even if I slept the recommended amount. I don't know if I'm just not trying to sleep or if I have a more serious problem.
