Long term mouth ulcers?

I have had several mouth ulcers concurrently for about 8 weeks. I saw an ENT guy who palpated the largest one, on the mouth floor, and declared it non-malignant. Smaller ones come/go and change position, even to the lips (but not like coldsores).

This is not a denture problem or a mouth cleaning problem.

The ENT guy gave me some B vits.

I have the feeling that this is systemic. Should I try to boost my immune system? I was HIV-neg at last check (but I do have checks often).

I have had a Roux-n-Y 2 years ago. I also take heart drugs and plavix after a single stent 1 year ago. I have OA of the ankles too.

Around 4 mths ago I had major gastroenteritis which was treated as Typhoid because the symptoms were borderline.

Can anyone describe systemic causes of oral ulcers?

Were they aphthous ulcers? B vitamin deficiency (B12 and folate) is a common cause; cow's milk has been implicated, as has gluten- many celiacs also have aphthous ulcers. Other causes include trauma, sometimes from foods that are hard and sharp like chips.

While this isn't getting at the cause, the detergents in toothpaste can sometimes aggravate this condition. Sodium lauryl sulfate is generally the strongest, but others can do it too; you might try switching to baking soda for a couple weeks and see if it helps; if it does, you can experiment with non-SLS-containing toothpastes.

But the roux en y definitely makes me wonder about nutrient deficiencies. A colleague recently mentioned the company Bariatric Advantage for supplements specifically formulated for bypass patients.
