    It is Fissure not fisher but here are a few things you can try before surgery.
    1. Increase fiber intake to keep stool soft. Get intake up to like 25-30 grams per day.
    2. Take laxatives - again to decrease constipation
    3. Sitz baths - This works by improving blood flow and relaxing the internal anal sphincter.
    4. Topical nitroglycerin — Nitroglycerin works to dilate blood vessels, increasing blood flow to injured tissues. It also works to reduce pressure in the internal anal sphincter, which decreases pain and further facilitates healing.
    5. Botulinum toxin (like in Botox and no it is not for wrinkles in this case) Botulinum toxin temporarily paralyzes muscles for up to three months, and can be injected into the internal anal sphincter muscle in a clinician's office without anesthesia or sedation.
    6. Nifedipine and Diltiazem manufactured into a gel and applied to rectum.

    Work on keeping your stool soft. (prunes, prune juice, stool softener, drink enough fluid, fruits & veggies, fiber. ) You get the picture. Next and really important, get some of those disposable wipes and use it after every bowel movement to keep the area as whistle clean as possible. If you enjoy the luxury of owning a sitz bath, the more power to you. Those hand held shower things are ideal too as you can use it to your advantage. wink Then make sure you keep the area dry. This usually solves the problem. It's difficult to heal this area as it is naturally unsterile. Just remember: Soft, Clean, Dry, Repeat.
