Typically in clinical practice I see Januvia decrease Hemoglobin A1c by only 0.4-0.6 points. Byetta commonly will decrease Hemoglobin A1c by 1-1.5 points. This is also dependant upon the height of the Hemoglobin A1c.
I usually see maximal weight loss of roughtly 5-15 lbs with my patients unless they are using other methods for weight loss. Januvia is weight neutral and does not add weight nor does it cause weight loss.
I see very few clinical side effects in most of my patients using Januvia. It is easy to take once per day and does not have to be timed with meals.
Byetta on the other hand must be timed with meals and is an injection twice daily. This is a significant hindrance to many that have "needle phobia." Byetta commonly causes nausea with most of my patients in the first two weeks of use, and for 30-40% of them, that nausea never totally resolves.
Therefore, in my clinical practice, Byetta is more effective in controlling blood sugars, however, Januvia is easier to use.
